The ASHA Board of Ethics found the following speech-language pathologists in violation of the association’s Code of Ethics. The listed violations refer to the version of the code and the specific rules and principles involved.
Kimberly D. Cooke Franklinton, Louisiana
  • Rationale: Received a 2016 Consent Agreement from the Louisiana Board of Examiners for Speech-Language Pathology and Audiology (LBESPA) revoking the respondent’s speech-language pathology license.

  • Violations (2010r) & (2016): I (2010r/2016), IV (2010r), IV-(2016)-A (2010r), IV-E (2010r), IV-D (2016), IV-R (2016), IV-T (2016)

  • Sanction: Withholding of membership and certification for five years, effective Jan. 24, 2017.

Kristi L. Pennypacker Claymont, Delaware
  • Rationale: Falsified home-visit records for four to five weeks while incapacitated due to medical conditions and failed to comply with the ASHA Ethics/Board of Ethics’ requests for documentation.

  • Violations (2010): I, I-O, I-Q, IV, IV-C, IV-N

  • Sanction: Censure, effective Jan. 18, 2017.

Ashley M. Sawyer Grimesland, North Carolina
  • Rationale: Documented and billed for speech-language pathology services for 14 children that were not rendered between January and October 2015.

  • Violations (2010r): I, I-O, III, III-D, III-E IV, IV-A, IV-C

  • Sanction: Revocation of membership and certification for five years, effective Jan. 18, 2017.

Kimberly R. Taylor Tulsa, Oklahoma
  • Rationale: Acted unprofessionally by failing to comply with employer policies, procedures, and supervision that are consistent with professional standards, and failed to provide mandated services.

  • Violations (2010r): I, IV, IV-A

  • Sanction: Censure, effective Jan. 18, 2017.

Karen M. Waldrop Miami Beach, Florida
  • Rationale: Failed to pay an independent contractor for the provision of speech-language pathology services and failed to respond to the complaint filed against her or the numerous follow-up correspondence from the ASHA Ethics/Board of Ethics.

  • Violations (2010r): IV, IV-A, IV-C, IV-N

  • Sanction: Revocation of membership and certification for 12 months, effective Jan. 13, 2017.

Enid E. Webb Gardnerville, Nevada
  • Rationale: Failed to comply with the federally protected rights of students with disabilities by not developing Individualized Education Programs or completing mandated documentation of the provision of IEP services, resulting in Medicaid billing errors. Also, removed IEP and confidential files from the school premises.

  • Violations (2010r): I, I-A, I-M, III, III-D, IV, IV-C

  • Sanction: Revocation of membership and certification for 12 months, effective Jan. 18, 2017.
