The ASHA Board of Ethics found the following ASHA members in violation of the association’s Code of Ethics. Unless otherwise indicated, the members are speech-language pathologists. The violations refer to the code version and the specific rules and principles involved; the date is when the sanction took effect.
Please note that beginning with the April 2017 issue, Board of Ethics decisions in the print version of The ASHA Leader will include the name, location and sanction (with effective date) of members found to have violated the ASHA Code of Ethics. Jocelynn A. Anderson, Medford, Oregon
  • Rationale: Practiced without a valid Oregon speech-language pathology license for more than one year (Feb. 1, 2014, to April 20, 2015).

  • Violations (2010r): III, III-A, III-F, IV, IV-A, IV-C.

  • Sanction: Censure (eff. 8-30-16).

Farah Brooke Cox, Edmond, Oklahoma
  • Rationale (resulting from two separate ethics complaints): Failed to obtain informed consent for services provided, conducted speech and language screenings without parental permission, and misrepresented in advertising by posting a sign advertising speech and language services as “free” when reimbursement was sought.

  • Violations (2010): I, I-H, III, III-G, IV, IV-A, IV-C, IV-E.

  • Sanctions: Censure (eff. 8-30-16).

Trina L. Elliott, Tulsa, Oklahoma
  • Rationale: Practiced speech-language pathology without a valid Oklahoma license for multiple weeks in two consecutive years.

  • Violations (2010): III, III-A, IV, IV-A, IV-C.

  • Sanction: Censure (eff. 8-12-16).

Kendra A. Grimes, Barnesville, Ohio
  • Rationale: Practiced without a valid speech-language pathology license for more than six months (Jan. 1 to July 8, 2015), resulting in a suspension by the Ohio Speech-Language Pathology and Audiology Board.

  • Violations (2010r): III, III-A, IV, IV-A, IV-C.

  • Sanction: Censure (eff. 8-31-16).

Kimber L. Johns, Cottage Grove, Oregon
  • Rationale: Convicted for driving while under the influence of alcohol and injuring another person.

  • Violations (2010r): IV, IV-E.

  • Sanction: Censure (eff. 8-30-16).

Thomas David Kehoe, Boulder, Colorado
  • Rationale: Provided information on his website and in literature that is likely to cause consumers to believe that his product is directly related to, or implies an endorsement relationship with, the published research of another individual.

  • Violations (2010r): III, III-G, IV, IV-A.

  • Sanction: Censure (eff. 8-25-16).

Aprill Richardson (audiologist), Moore, Oklahoma
  • Rationale: Referred to a patient and family member in a derogatory manner in writing.

  • Violations (2010): I, IV, IV-A, IV-E.

  • Sanction: Censure (eff. 8-23-16).

James Fletcher Scott, Bellingham, Washington
  • Rationale: Permitted content changes to an evaluation report by an office manager who is not a speech-language pathologist and misrepresented to his clinical fellow the requirements for direct clinical contact during a clinical fellowship.

  • Violations (2010r): I, I-E, IV, IV-A, IV-C.

  • Sanction: Censure (eff. 8-31-16).

Karen I. Selig, Fair Oaks, California
  • Rationale: Allowed excessive delays in providing the therapy records documenting services provided and in returning equipment to her employer.

  • Violations (2010r): I, I-M, IV, IV-A.

  • Sanction: Censure (eff. 9-2-16).

Lyna L. Smith, Powell, Ohio
  • Rationale: Admitted to the Ohio Board of Speech-Language Pathology and Audiology that she consumed alcohol prior to arriving at her place of employment.

  • Violations (2010r): I, I-Q, IV, IV-A, IV-E.

  • Sanction: Censure (eff. 8-12-16).
